ISMP, ProCE and Sanofi gave a symposium at the 50th ASHP Midyear Clinic on Dec. 8, 2015. The above slide was used. The symposium was entitled "Concentrated Insulin: Safe Use in the Hospital and Transitions of Care." 

The slide title is "Products Most Frequently involved in Harmful Medication Errors (2006)".

Insulin appears at the top of the list.
